Consent to Treatment and Liability Waiver:
By participating in services provided by HIDEF Physical Therapy at this event, including but not limited to NormaTec compression therapy, use of massage guns, stretching, and manual therapy, I acknowledge and accept the inherent risks associated with these treatments. I confirm that I do not have any medical conditions that would make such services unsafe. I voluntarily consent to treatment and agree to release HIDEF Physical Therapy, its staff, and affiliates from any and all liability for injury or adverse outcomes resulting from participation in these services.
